Marfan症候群の毛様体所見

Ultrasound biomicroscopic findings of ciliary body in Marfan syndrome

　Marfan症候群の3症例に対し，超音波生体顕微鏡にて毛様体部の観察を行った。症例は，術後無水晶体眼4眼，水晶体完全脱臼1眼，水晶体亜脱臼1眼の合計6眼であり，正常眼と比較すると6眼すべてで毛様体の平坦化および毛様体突起の短縮を認めた。23歳時に経毛様体扁平部水晶切除術を施行された症例では，術後6年経過していたが，平坦化が比較的軽度であり，突起も他の5眼に比べ比較的明瞭に描出された。一方，水晶体切除術を施行された先天白内障の症例は術後30年経過していたが，平坦化，突起短縮の所見はなく，Marfan症候群では毛様体襞部の平坦化と突起部短縮という毛様体の低形成が特徴的な所見であることが示唆された。

We examined 6 eyes in 3 cases of Marfan syndrome using an ultrasound biomicroscope. Four eyes had postsurgical aphakia, one had lenticular dislocation, and one had lenticular subluxation. All the eyes showed flattening of the ciliary body and shortening of ciliary processes. These features were less pronounced in one eye which had received pars plana lensectomy 6 years before. Another case without Marfan syndrome, who had received surgery for congenital cataract 30 years before, showed normal configuration of the ciliary body. We postulate that flattening of the ciliary body and shortening of ciliary processes are characteristic features of Marfan syndrome.
