術後の瞳孔領フィブリン膜に対する組織型プラスミノーゲンアクチベーター前房内注入

Intracameral injection of tissue plasminogen activator for treatment of postoperative pupillary fibrin formation

　内眼手術後の瞳孔領フィブリン膜形成に対し組織型プラスミノーゲンアクチベーター（t-PA）の濃度を変えて前房内注入を施行し，その結果を比較検討した。注入時期およびt-PA注入量は，術後4〜7日目に25μg／0.1ml，注入を行っているのに対し，術翌日から術後4日目に3μg／0.1ml，注入を施行した。フィブリン溶解に要する時間は両群の間に差は見られず，フィブリン完全溶解は3μg注入群が5例中4例，25μg注入群が全例にみられた。副作用は，3μg注入群ではなかったが，25μg注入群の2例に前房出血とフィブリン再析出がみられた。術後の瞳孔領フィブリン膜の溶解には，3μg／0.1ml，t-PAが副作用もなく十分なフィブリン溶解能を有し，術後早期から前房内に注入でき有用な治療法である。

We treated 12 eyes with fibrinous pupillary membrane after intraocular surgery by injecting tissue plasminogen activator into the anterior chamber. The dose was set either at 25μg or 3μg per 0.1ml. The higher dose was injected in 7 eyes on day 4 to 7 after surgery. The lower dose was injected in 5 eyes on day 1 to 4 after surgery. Fibrinolysis resulted 5 to 45 minutes after injection regardless of the concentration. Complete fibrinolysis resulted in all the 7 eyes in the high-dose group and in 4 of 5 eyes in the low-dose group. Hyphema with reaccummulation of fibrin developed in 2 of 7 eyes in the high-dose group. No side effect developed in the 5 eyes of low-dose group. The findings indicated that intracameral injection of tissue plasminogen activator is effective and safe for postoperative fibrin formation at the concentration of 3μg/0.1ml.
