偽水晶体眼の増殖糖尿病網膜症の硝子体手術

Vitrectomy for pseudophakic proliferative diabetic retinopathy

（18-D501-33）　1993年6月〜1997年2月に福岡大学病院眼科で硝子体手術を行った偽水晶体眼の増殖糖尿病細膜症（PDR）17例20眼において，最終的に眼内レンズ（IOL）を保存した14眼とIOLを摘出した6眼を比較した。両群ともに網膜復位，病変鎮静化，眼圧のコントロールが得られた。その結果から，偽水晶体眼の増殖糖尿病網膜症の硝子体手術においては虹彩ルベオーシス，血管新生緑内障の眼球前半部の十分な処置が必要な場合，高度の後発白内障により眼底の透見が困難な場合，手術施行時にIOLが脱落する可能性がある場合にはIOLを摘出するのがよいと考えた。

We reviewed 20 pseudophakic eyes which underwent vitrectomy for proliferative diabetic retinopathy. The intraocular lens (IOL) was kept in place in 14 eyes and was removed in 6 eyes. Both groups showed gratifying outcome regarding retinal reattachment, regression of retinopathy and control of intraocular pressure. We advocate removal of IOL during vitrectomy in the presence of rubeosis iridis, nuovascular glaucoma, dense aftercataract and possibility of intrasurgical IOL luxation.
