糖尿病黄斑浮腫に対する硝子体手術の長期予後

Long-term results of vitrectomy for diabetic macular edema

（26B2-3）　糖尿病黄斑浮腫に対ずる硝子体手術の結果をretrospectiveに検討し，その有効性，手術適応について検討した。対象は，1989〜1995年の期間に，当科にて手術が施行された15例20眼で，経過観察期間は，6〜55か月，平均20.8か月であった。術前，検眼鏡的に後部硝子体剥離が無く，後部硝子体膜の肥厚を伴い，硝子体牽引の関与が強く示唆されることを手術適応とした。術前と術後，最終受診時視力を比較したところ，12眼（60％）で2段階以上の改善，6眼（30％）で不変，2眼（10％）で2段階以上の悪化が認められ，有意な改善を認めた（p＜0.01）。平均視力は，術後2年後まで経過期間が長くなるほど上昇していた。悪化症例中1眼は1黄斑部への硬性白斑の集中した症例，他の1眼は，術後，網膜剥離や血管新生緑内障をきたした症例であった。糖尿病黄斑浮腫に対する硝子体手術は，長期間経過しても有効であるが，硝子体手術に伴う重篤な合併症により，著明に視力低下をきたした症例もあり，より慎重な手術適応の検討が不可欠である。

We reviewed clinical courses of 20 eyes of diabetic macular edema treated by vitrectomy. The follow-up periods ranged from 6 to 55 months, average 20.8 months. Eyes were judged as necessitating vitrectomy when no posterior vitreous detachment was present, the posterior hyaloid membrane appeared thick and when vitreous traction was highly suspected. At the final examination, the visual acuity improved by 2 lines or more in 12 eyes (60%), remained unchanged in 6 (30%) and deteriorated by 2 lines or more in 2 (10%) . The improvement in visual acuity was significant (p ＜ 0.01) . The averaged visual acuity continued to improve till 2 years after surgery. Deterioration in visual acuity was due to accumulation of hard exudates in the macula in one eye and to retinal detachment and neovascular glaucoma in another. The findings show the efficacy of vitrectomy for diabetic macular edema over longer periods. Due precaution is necessary in deciding surgery because of possible severe complications after surgery.
