We performed vitrectomy in 7 eyes of diabetic maculopathy to remove persistent and massive subfoveal hard exudates. The postoperative follow up ranged from 6 to 46 months, average 22 months. The hard exudates were confined to the subfoveal area in 4 eyes and involved a wider area inner to the vascular arcades in 3 eyes. The initial visual acuity was 0.1 or less in all the eyes. Postoperatively, the visual acuity improved by 2 lines or more in 6 eyes. The best visual acuity was 0.2 among the whole series. Surgical complications included macular hole 4 eyes, cataract 2 eyes and epiretinal proliferation 1 eye. The relatively poor visual outcome appeared to be due to impaired neural retina secondary to hard exudates in general and to tight adhesion of the subfoveal exudates leading to retinal defect during surgery. Surgical removal of subfoveal hard exudates in diabetic maculopathy may be indicated in eyes with very low visual acuity.
