We reviewed 69 eyes which showed elevated intraocular pressure (IOP) over 22 mmHg following pars plana vitrectomy. The series included proliferative diabetic retinopathy 42 eyes (62%). The maximal IOP value ranged from 22 to 29 mmHg 42%, 30 to 39 mmHg 30% and over 40 mmHg 28%. The elevation of IOP was due to secondary open angle glaucoma 43%, ghost cell glaucoma 14%, neovascular glaucoma 19%, secondary angle closure glaucoma 16% and gas expansion 7%. Eyes with neovascular glaucoma and secondary angle closure glaucoma showed higher maximum IOP and longer period of elevated IOP than secondary open angle glaucoma. Use of silicone oil, sclera] buckling and aphakia contributed to elevated IOP. The final visual outcome was dependent on the maximum TOP level.
