　イブジラストは，プロスタサイクリンの作用を増強し，血管弛緩作用，抗血小板作用を呈する。筆者らは，糖尿病のような血栓形成を伴う血管障害の治療に有用ではないかと考え，video—densitometric image analysisの方法を用いて，網膜循環動態への効果を検討した。

　対象症例は，糖尿病患者8例8眼で，イブジラストの内服前と30mg／日を2週間内服後にビデオ螢光眼底造影を行い，網膜平均循環時間（MCT）を計測した。内服後では，MCTは有意に短縮した（前4.2±2.8秒vs後3.0±1.6秒，P＝0.0215）。イブジラストによる網膜循環の改善効果は，糖尿病網膜症の進行防止や予防の有用性を示唆する。

We evaluated the effect of perorally administer-ed ibudilast on the dynamics of retinal circulation in 8 diabetic patients with absent or moderate nonproliferative retinopathy. Ibudilast is supposed to induce cerebral vasodilatation and to prevent platelet aggregation through enhancement of pros-tacyclin. With the use of video-densitometric image analysis by fluorescein angiography, we measured the mean retinal circulation time (MCT) before and after administration of ibudilast at the daily dosis of 30 mg for 2 weeks. The MCT averaged 4.2±2.8 sec before and 3.0±1.5 sec after treatment. The difference was significant (p=0.0215). The finding implies that systemic ibudilast is of poten-tial value in treatment of diabetic retinopathy through acceleration of retinal circulation.
