反復性前房出血を伴える網膜膠腫

Retinoblastoma with Recurrent Bleedings into the Vitreous and Anterior Chamber

緒言

　網膜膠腫の診断は困難なことがある。化膿性眼内炎が硝子体膿瘍を起こし，眼底に黄白色の腫瘍状の隆起を生ずる偽神経膠腫と鑑別出来ないことがあり，又水晶体後面結合織増殖症，硝子体内生来結締織遺残，滲出性網膜炎，梅毒，結核，脈絡膜肉腫とも鑑別を要する。網膜膠腫による網膜剥離，続発性緑内障，白内障に注目するあまりその原因である膠腫に考えの及ばない例もある。網膜膠腫で早期に前房蓄膿を現わし，或いは虹彩に結節を生じ，結核性虹彩毛様体炎の症状を呈した報告も多く，前房蓄膿様出血を来たした例もある。私は初期より多量の眼内出血を反復した網膜膠腫例を経験した。これらに於いては定型的病像が隠蔽され，確定診断が遅らされ，治療の期を逸することがあるので注意すべきであり，敢えて報告する次第である。

A case of retinoglioma of which diagnosis was masked by symptoms of a panophalmitis and then recurrent bleedings into the vitreous and anterior chamber, was reported.

For the determination of diagnosis, 100 microcuries of P³² was injected intravenously resulting the counts of P³² of The tumor eye was higher than that of the other.

Histologically, it was a retinoblastoma with an incomplete rosette formation filling whole eye globe and extending into the anterior orbit.
