眼内レンズ二次縫着後に発症した遅発性眼内炎の1例

A case of late-onset endophthalmitis after secondary intraocular lens implantation by suturing

　水晶体嚢内摘出術が9年前に行われている79歳の女性に眼内レンズ二次縫着術を施行した。術後6週目に，激しい前房蓄膿と眼内炎が突発した。抗生物質の局所および全身投与で治療を開始したが，炎症が増悪し，著しい硝子体混濁が生じたため，治療開始後1週間目に前房洗浄および硝子体手術を施行し，比較的良好な視力回復を得られた。

　本症例では硝子体手術時に検体を採取できず，原因菌の同定はできなかったが，臨床症状から弱毒菌による眼内炎が疑われた。眼内レンズの二次縫着術後にもこのような遅発性の眼内炎が発症する可能性があるので，前房および硝子体手術標本を好気性のみならず嫌気性菌の培養と細胞学的検査を行う必要がある。

We performed secondary intraocular lens im-plantation with suturing by 9-0 prolene in the left eye of a 79-year-old female. Intracapsular cataract extraction had been performed 9 years before. Hypopyon started 6 weeks after the secondary surgery. The condition developed into persistent iridocyclitis with hypopyon and vitreous opacity. Topical and systemic treatment with antibiotics proved ineffective. Pars plana vitrectomy induced cure and recovery of vision. Culture of aqueous humor during surgery proved negative. The clinical findings were suggestive of so-called localized endophthalmitis.
