超音波乳化吸引術による術後角膜倒乱視例の解析（その2）

Corneal astigmatism after phacoemulsification cataract surgery

　白内障の手術成績を単純化した条件で検討することを目的として，同一時期，同一術者，同一方法によってなされた白内障手術を検討した。

　総数1,060眼で，KPE809眼（76.3％），ECCE217眼（20.5％），その他34眼（3.2％）である。そのうち，6か月間追跡できた症例は586眼で，KPE 463眼（79.0％），ECCE 104眼（17.7％），その他19眼（3.3％）であった。

　KPE法に限定し，術前および術後の角膜倒乱視を解析すると，術後倒乱視度数≦0.7D以上では，術後1週での直乱視化の度数が著しく上昇した（2.66±0.17D）。また，術前が直乱視であろうと倒乱視であろうと，術後6か月のほうが乱視度は減少した。ところが術後倒乱視度数≧1.5Dでは，術後1週での直乱視化（2.12±0.13D）が弱く，3か月で一定せず6か月になっても倒乱視度に増加がみられた。弱主経線が術前値にもどらないことが原因であることがわかった。

We evaluated the corneal curvature during thefirst 6 months after cataract surgery in 463 eyeswith against-the-rule postoperative astigmatism.These eyes were from a larger group of 1,060 eyesthat underwent cataract surgery. Surgery was per-formed by one surgeon with the same surgicalprocedure using 10-0 mersilene suture in double-running fashion.

In cases of against-the-rule postoperative astigmatism of 0.7D or less, a pronounced shift by 2.66±0.17D occurred during the first week. In cases ofastigmatism of 1.5D or greater, the shift was smal-ler by 2.12±0.13D. The degree of against-the-ruleastigmatism increased even 6 months after surgery.The findings seemed to suggest the postoperativechanges of inverse astigmatism in dependent uponthe absolute value of astigmatism.
