　23歳男性が，5か月前からの右眼霧視で受診した。眼圧は右51mmHg，左21mmHgで，両眼の隅角にSchwalbe線の肥厚と虹彩の高位付着があり，両眼の発達緑内障と診断した。右眼角膜の輪部付近に帯状の菲薄とその上方部の突出があり，角膜曲率計で8.75Dの乱視があり，右眼のペルーシド角膜変性症と診断した。右眼への濾過手術で眼圧は正常化した。角膜乱視は術後軽減したが，角膜形態に変化はなかった。

A 23-year-old male presented with blurring of right eye since 5 months before. The intraocular pressure (IOP) was 51mmHg right and 21mmHg left. Both eyes showed thickening of ring of Schwalbe and high insertion of the irits. He was diagnosed with bilateral developmental glaucome. His right cornea showed band-like thinning in its inferior portion with protrusion more superiorly. Keratometry showed astigmatism of 8.75 diopters. The right eye was diagnosed with pellucid marginal corneal degeneration. Filtering surgery induced normalization of IOP in the right eye. Postsurgically, the corneal astigmatism decreased with no change in the shape of the cornea.
