A 36-year-old Japanese male presented with chronic open angle glaucoma, heavy pigmentation in the chamber angle and myopia in both eyes. Filtering surgery had been performed on the right eye before. These findings led to the diagnosis of pigmentary glaucoma in spite of absence of Krukenberg spindles. Ultrasound biomicroscopy showed concave configuration of the left iris suggesting the presence of reverse pupillary block. The iris became flat after laser iridotomy. This case illustrates that pigmentary glaucoma may be present, although very rarely, in the Japanese population.
