レーザー虹彩切開術を施行した色素緑内障の1例

A case of pigmentary glaucoma treated with laser iridotomy

　本邦においては稀な続発緑内障とされる色素緑内障の1例を経験した。超音波生体顕微鏡により虹彩周辺部の眼球後極部へ向かう屈曲がみられたため，reverse pupillary blockを解消する目的でレーザー虹彩切開術を施行した。レーザー虹彩切開術後には虹彩は平坦化し，reverse pupillary blockが解消されたことが確認された。色素緑内障に対するレーザー虹彩切開術は，その発症原因と考えられる虹彩形態の異常を解消する上で有効なこと，ならびに，虹彩形態異常の検出に超音波生体顕微鏡が有用なことが確認された。

A 36-year-old Japanese male presented with chronic open angle glaucoma, heavy pigmentation in the chamber angle and myopia in both eyes. Filtering surgery had been performed on the right eye before. These findings led to the diagnosis of pigmentary glaucoma in spite of absence of Krukenberg spindles. Ultrasound biomicroscopy showed concave configuration of the left iris suggesting the presence of reverse pupillary block. The iris became flat after laser iridotomy. This case illustrates that pigmentary glaucoma may be present, although very rarely, in the Japanese population.
