強度近視眼に合併した網膜分離症

Optical coherence tomographic features of retinoschisis in high myopia

　光学的干渉断層計で，強度近視（37眼）の網膜断層像を観察した。後部ぶどう腫のない5眼では，網膜は正常の厚さと層構造を示した。後部ぶどう腫の浅い4眼では，網膜は正常の層構造を示したが，厚さが減少していた。後部ぶどう腫が深い28眼中11眼では黄斑に網膜分離があり，うち9眼では中心窩に限局した網膜剥離を伴っていた。残りの17眼では網膜分離はなかったが，網膜の菲薄化と層構造の消失があった。後部ぶどう腫眼では，網膜分離症と網膜剥離が高頻度に生じていることが明らかになった。

Optical coherence tomography (OCT) was performed on 37 high myopic eyes. The sensory retina showed normal thickness and layered pattern in 5 eyes without posterior staphyloma.The retina showed normal layered pattern and decreased thickness in 4 eyes with shallow posterior staphyloma. Retinoschisis was present in the macula in 11 out of 28 eyes with deep posterior staphyloma. Subclinical macular detachment was present in 9 of these 11 eyes. In the remaining 17 eyes, the sensory retina was attenuated with absent layered pattern. The findings show that retinoschisis with or without macular detachment is a frequent occurrence in high myopia with deep posterior staphyloma.
