　眼サルコイドーシス4例5眼に対してメトトレキサート7.5mgを週1回，6か月以上投与した。全例がステロイド薬が無効な重症であるか，副作用でステロイド薬の投与継続が不可能であった。視力は2眼で不変，3眼でやや低下した。嚢胞様黄斑浮腫と血管炎は5眼中2眼で改善し，新生血管は3眼中3眼で改善した。ステロイド薬は全例で投与不要または減量が可能になった。副作用として，胃部不快感と間質性肺炎の疑いが各1例あった。少量の経口メトトレキサートが重症眼サルコイドーシスに有効である可能性があるが，適応と用法についてはさらに検討が必要である。

We treated 5 eyes of 4 patients of ocular sarcoidosis by peroral methotrexate at the dosis of 7.5 mg once aweek for 6 months or longer. All the cases had cystoid macular edema (CME) and/or retinal neovascularization resistantto corticosteroids. After treatment, the visual acuity improved in 2 eyes and slightly decreased in 3 eyes. CME and retinal vasculitis improved in 2 of 5 eyes. Retinal neovascularization improved in 3 of 3 eyes. Systemic corticosteroid could bediscontinued or reduced in dosage in all the cases. Gastric irritation and interstitial pneumonia developed in one case each. The findings show that peroral methotrexate may be effective for severe ocular sarcoidosis with CME and/or retinalneovascularization. Further studies are necessary to determine the indication and dosage.
