繊細な手術操作を必要としない内境界膜剥離方法

A simple technique for internal limiting membrane peeling

　櫻井らの受動吸引による内境界膜剥離方法を改変した。エクストルージョンニードルのシリコンチューブの先端を少し斜めに切断し，吸引口と網膜表面の隙間を少なくすることにより内境界膜を効率的に吸着できるようにしたものを作製し使用した。本方法ではシリコンチューブを伸ばせば金属性の外筒を網膜に近づけなくても吸引できるため，ブラシ付きバックフラッシュニードルに比較して繊細な操作を必要とせず，網膜損傷の危険性が少ないと思われた。

We devised a new technique for peeling of internal limiting membrane (ILM) during vitreous surgery. It is a modification of passive suction ILM peeling method by Sakurai et al. Ours uses an improved sliding extrusion needlewith an obliquely cut tip. This reduces the gap between the orifice of the tip and the retinal surface. ILM thus clings tothe orifice efficiently even when the metallic outer sleeve lies away from the retina. The present device of ILM peeling requires no delicate manipulation and would reduce potential damage to the retina.
