A 74-year-old female noted blurring of vision in her right eye 4 weeks before . The visual acuity in the affected eye was finger counting at 30cm. Funduscopy showed subretinal hemorrhage and a hematoma posterior to the internal limiting membrane (ILM) of 3 disc diameters. Fluorescein angiography led to the diagnosis of ruptured retinal arteriolar macroaneurysm. The affected eye underwent vitrectomy with ILM peeling and aspiration of hematoma 2 weeks later. The submacular hemorrhage was left intact because of its extrafoveal location. The visual acuity improved to 0.7 with slight macular degeneration 3 months later. This case illustrates that ILM peeling with aspiration of sub-ILM hematoma may be effective for ruptured retinal macroaneurysm.
