内境界膜剥離術を施行した網膜細動脈瘤による内境界膜下血腫の1例

A case of retinal macroaneurysm with hemorrhage posterior to the internal limiting membrane treated by membrane peeling

　74歳女性が右眼視力低下を4週間前に発症した。右眼視力は30cm指数弁であり，黄斑部に3乳頭径大の内境界膜下血腫と網膜下出血があった。蛍光眼底造影所見などから網膜細動脈瘤破裂による出血と診断した。その2週後に硝子体手術を行い，内境界膜を剥離して血腫を吸弓除去した。網膜下出血は中心窩外にあったので放置した。黄斑部に萎縮が残ったが，3か月後に0.7に改善した。内境界膜を剥離して血腫を吸引除去する手技が本症に有効であることを示す症例である。

A 74-year-old female noted blurring of vision in her right eye 4 weeks before . The visual acuity in the affected eye was finger counting at 30cm. Funduscopy showed subretinal hemorrhage and a hematoma posterior to the internal limiting membrane (ILM) of 3 disc diameters. Fluorescein angiography led to the diagnosis of ruptured retinal arteriolar macroaneurysm. The affected eye underwent vitrectomy with ILM peeling and aspiration of hematoma 2 weeks later. The submacular hemorrhage was left intact because of its extrafoveal location. The visual acuity improved to 0.7 with slight macular degeneration 3 months later. This case illustrates that ILM peeling with aspiration of sub-ILM hematoma may be effective for ruptured retinal macroaneurysm.
