（18-P1-20）　過去29か月の間に，弁状裂孔網膜剥離111例111眼に手術を行った。初回手術は，硝子体手術32眼，強膜内陥術79眼である。初回復位率は，それぞれ97％と89％であり，最終的には全例が復位した。主な合併症は，硝子体手術では術中の医原性裂孔形成と術後の白内障の進行であり，強膜内陥術では黄斑前膜であった。硝子体混濁が強い例，裂孔が渦静脈またはそれよりも後方にある例では，初回手術として硝子体手術が適当であると結論される。

Surgery was performed on 111 eyes of rhegmatogenous retinal detachment during the foregoing 29 months. Vitrectomy was performed as the initial surgery on 32 eyes and scleral buckling on 79 eyes. Initial retinal reattachment was obtained in 97% and 89% of eyes respectively. Final reattachment was obtained in all the 111 eyes. Major complications of vitrectomy included intraoperative iatrogenic retinal tear and postoperative progression of cataract. Macular pucker was the major complication of scleral buckling procedure. Initial vitrectomy is recommended for eyes with dense vitreous opacity and for eyes with retinal tear located posterior to the vortex vein.
