Purpose: To evaluate the effect of removal of the internal limiting membrane with gas tamponade for cystoid macular edema. Cases and Method: Vitreous surgery was performed on 12 eyes by removing the internal limiting membrane and tamponade using SF6 gas. The series comprised diabetic retinopathy 7 eyes, central retinal vein occlusion 4 eyes and branch retinal vein occlusion 1 eye. Cystoid macular edema had been present for 6 months or longer in spite of laser photocoagulation. Posterior vitreous detachment was present in 5 eyes at the time of surgery. Results: Cystoid macular edema decreased or disappeared in 11 eyes 1 to 7 months after surgery. Optical coherence tomography showed decrease in edematous swelling in outer retinal layers associated with flattening or disappearance of intraretinal cysts. Visual acuity improved by 2 lines or more in 7 eyes, particularly when surgery was performed within 11 months after presumed onset of cystoid macular edema. Conclusion: The present surgical modality was effective for cystoid macular edema secondary to diabetic retinopathy or retinal vein occlusions.
