　超音波水晶体乳化吸引術で残留皮質を吸引したのち，内視鏡を前房に挿入して観察すると，シュレム管に血液が充満する発赤現象が検索した20眼すべてで起こっていた。この現象を利用して，白内障と緑内障の同時手術時に，内視鏡による直視下で内方線維柱帯切開術trabeculotomy ab internoを2名3眼に行った。術後，一過性に眼圧が上昇したのち，全例で眼圧のコントロールが得られた。この方法は，簡便で有効な手技である。

We observed the chamber angle by inserting the tip of an endoscope into the anterior chamber after aspiration of residual cortex during phacoemusification-aspiration for cataract. Blood-filling of the canal of Schlemm was present in all 20 eyes thus examained. We performed trabeculotomy ab externo in 3 eyes of 2 patients during simultaneous surgery for cataract and glaucoma under endoscopic observation. The intraocular pressure (IOP) was brought under control following a temporary TOP elevation in all the eyes. This technique is advocated for its simplicity and effectiveness.
