要約　目的：網膜前膜（ERM）と緑内障を合併する白内障患者に対し，白内障手術に，マイクロフックを用いた線維柱帯切開術（眼内法）を併施（以下，phaco＋μLOT）後，黄斑浮腫（ME）が発症・遷延し，視力が低下した2症例3眼を報告する。

症例1：74歳，女性。両眼に白内障と開放隅角緑内障（OAG）があり，左眼はstage 3 ERM（＋），術前矯正視力1.2，眼圧11mmHgであった。phaco＋μLOT後21日にMEを認め，矯正視力は0.6に低下した。点眼治療にてMEは軽度改善したが，視力は回復しなかった。

症例2：82歳，女性。両眼に白内障とOAGがあり，右眼はstage 3 ERM（＋），術前矯正視力0.9p，眼圧16mmHgで，左眼はstage 1 ERM（＋），術前矯正視力1.0，眼圧18mmHgであった。両眼にphaco＋μLOTを施行し，右眼は術後29日，左眼は術後17日にMEを認め，矯正視力は右0.3，左0.9と低下した。点眼治療により左眼のMEは改善したが，右眼では改善せず，トリアムシノロンTenon囊下注射施行後もMEは残存し，視力は回復しなかった。

結論：ERM合併緑内障・白内障症例に対しphaco＋μLOTを行う場合には，術後にMEが発症して視力が低下する可能性について，術前に患者説明を行う必要がある。

Abstract　Purpose：This case report presents two cases of cataract with epiretinal membrane（ERM）and glaucoma wherein persistent macular edema（ME）and vision deterioration were observed following phacoemulsification combined with microhook-assisted trabeculotomy（phaco＋μLOT）.

Case 1：Case 1 was a 74-year-old woman with bilateral cataracts and open-angle glaucoma（OAG）and stage 3 ERM in the left eye. The pre-operative best-corrected visual acuity（BCVA）and intraocular pressure（IOP）in the left eye were 1.2 and 11 mmHg, respectively. ME was observed in the left eye 21 days after phaco＋μLOT, with the BCVA decreasing to 0.6. Slight improvement in ME was observed with eye drop treatment；however, visual acuity remained unchanged.

Case 2：Case 2 was an 82-year-old woman with bilateral cataracts and OAG. The right eye had stage 3 ERM, with a pre-operative BCVA and IOP of 0.9p and 16 mmHg, respectively. The left eye had stage 1 ERM, with a pre-operative BCVA and IOP of 1.0 and 18 mmHg, respectively. ME was observed in both eyes after undergoing phaco＋μLOT, with visual acuity decreasing to 0.3 and 0.9 in the right and left eyes 29 and 17 days postoperatively, respectively. ME in the left eye improved with eye drop treatment. ME in the right eye remained unresolved despite triamcinolone injection, and visual acuity did not recover.

Conclusion：The potential risk of postoperative ME and vision loss must be explained pre-operatively to the patients with ERM complicated by glaucoma and cataract undergoing phaco＋μLOT.
