白内障併用全周線維柱帯切開術の術後短期成績

Short-term outcome of simultaneous 360-degree trabeculotomy and cataract surgery

要約　目的：白内障手術と併用した全周線維柱帯切開術の短期成績の報告。対象と方法：白内障手術と360°線維柱帯切開術を同時に行った20例26眼を対象とした。男性15眼，女性11眼で，平均年齢は73歳である。緑内障の病型は，落屑緑内障12眼，原発開放隅角緑内障10眼，慢性閉塞緑内障4眼である。術後9か月以上の経過を観察し，眼圧と投薬スコアを評価した。結果：全症例での平均眼圧は，術前17.8±4.9mmHg，9か月後12.0±3.1mmHgで，有意に眼圧が下降した。全症例での投薬スコアは術前の2.8±1.1点から術後0.9±0.7点に有意に下降した。術中または術後に重篤な合併症はなかった。眼圧下降と投薬スコアの減少は，落屑緑内障と原発開放隅角緑内障で有意であった。結論：白内障手術と併用した全周線維柱帯切開術は安全な術式であり，落屑緑内障と原発開放隅角緑内障に有効であった。

Abstract.　Purpose：To report the short-term outcome of simultaneous 360-degree trabeculotomy and cataract surgery. Cases and Method：This study was made on 26 eyes of 20 cases who received simultaneous 360-degree trabeculotomy and cataract surgery. The series comprised 15 male and 11 female eyes. The age averaged 73 years. There were exfoliation glaucoma 12 eyes, primary open-angle glaucoma（POAG）10 eyes, and chronic closed angle glaucoma 4 eyes. Cases were evaluated regarding intraocular pressure（IOP）and medication score for nine months after surgery. Results：IOP in 26 eyes averaged 17.8±4.9 mmHg before surgery and 12.0±3.l mmHg nine months after surgery. Medication score in 26 eyes averaged 2.8±1.1 before surgery and 0.9±0.7 nine months after surgery. Decrease in IOP and medication score was significant in eyes with exfoliation glaucoma or POAG. There was no serious complication during or after surgery. Conclusion：Cataract surgery with simultaneous 360-degree trabeculotomy was a safe procedure and was effective for exfoliation glaucoma and POAG.
