Axenfeld-Rieger症候群の1症例

A case of Axenfeld-Rieger syndrome

　15歳男子で，両眼に角膜拡大，後部胎生環，iris strandが認められ，緑内障を合併したAxenfeld-Rieger症候群（Axenfeld-Rieger syndrome：ARS）の1症例を経験した。一般的にARSによる緑内障は難治性であり，複数回の手術を要するとされている。本症例では術前から緑内障性視野障害が進行していたため，両眼の線維柱帯切開術を施行し，術後は点眼薬2種にて眼圧コントロール可能となった。ARSが疑わしい症例では，眼科的に後部胎生環やiris strandなどを検索し，定期的に眼圧を測定することが必要であり，高眼圧がみられた場合は，線維柱帯切開術を含めた早期治療を行うべきであると考えられた。

A 15-year-old boy presented with glaucoma, corneal ectasia, posterior embryotoxon and iris strands in both eyes, leading to the diagnosis of Axenfeld-Rieger syndrome. He underwent trabeculotomy in both eyes due to advanced glaucomatous visual field defect. After surgery, the intraocular pressure was kept under control with instillation of two antiglaucomatous agents. This case illustrates that early trabeculotomy may be effective in this syndrome in the presence of elevated intraocular pressure.
