コンタクトレンズ装用による角膜内皮障害と安全基準

Damage to the corneal endothelium following contact lens wear and aproposal of safety guidelines

（P2-1-28）　55歳以下で他の眼疾患を伴わないコンタクトレンズ装用者（1,369眼686例）の角膜内皮細胞障害につき検討した（ハードコンタクトレンズ891眼，ソフトコンタクトレンズ478眼）。細胞密度が2,000cells／Mm²以下の例を6例10眼認め，ソフトコンタクトレンズ，ハードコンタクトレンズそれぞれの典型例を供覧した。分析については単純化するために，障害程度は細胞密度のみで評価し，装用期間，1日平均使用時間の2項目につき検討した。装用期間については内皮障害との相関がなかった。1日平均装用時間も直接的相関をみなかったが，細胞密度2,500ce〜ls／mm²の症例については1日装用時間12時間以上と以下の群に有意差を認めた（pく0.001）。また，12時間以下の症例には2,500cells／mm²以下の例はなかった。コンタクトレンズ装用者は定期的に内皮検査をするべきであり，1日装用時間を12時間以下にとどめるのが望ましい。

I evaluated the corneal endothelium in 1,369 contact lens wearers. They were under 55 years of age and were free of eye diseases. Conventional hard and soft contact lens was used by 891 and 478 eyes respectively. Decrease in corneal endothelial cells below 2,000 cells/mm² was present in 10 eyes of 6 users. The incidence of endothelial damage was correlated neither with the length of history nor with the daily duration of contact lens wearing. There was a significant correlation between corneal endothelial cells below 2,500 cells/mm² and daily duration of contact lens wear exceeding 12 hours (p＜0.001). There was no eye with corneal endothelial cells below 2,500 cells/mm² when the daily duration of contact lens wear was less than 12 hours. The findings indicate that contact lens wearers have to be examined periodically for corneal endothelial cells and that the daily duration of contact lens wear must not exceed 12 hours.
