長期コンタクトレンズ装用の角膜内皮に及ぼす影響—2．変化の進み方について

Effect of longterm contact lens wear on the corneal endothelium. :2. Mode of progression of endothelial lesions

　コンタクトレンズ（CL）長期装用者（平均14.6年），39症例74眼の角膜内皮細胞をspecular microscopeで観察し，日本人正常者平均値^1）より，1SD以上と2SD以上ずれていた症例を抜き出し，その変化の進行の仕方につき検討した。その結果，角膜内皮の変化は，まず，変動係数（CV）の変化として現れ，次いで，CVの変化とともに平均細胞面積の増大がみられた。しかし，CL装用期間が長くなり，平均細胞面積が400μm²以上になると，CVの値は，かえって小さくなった。すなわち，CL装用による角膜内皮の形態的変化は次のような段階で進行するものと考えられた。1）大細胞が散在して出現する初期の段階，2）大細胞が数個連なり散在する段階，3）大細胞群，小細胞群と群れをなす段階，そして最後に，4）大細胞がほとんどを占め，小細胞が散在し残在する段階。

We evaluated the corneal endothelial cells in 74 eyes of 39 longterm contact lens wearers, average 14.6 years. Lesions in the corneal endothelium became initially manifest as increase in the coeffi-cient of variation (CV), followed by increase in mean cell area. The CV value became again smal-ler in more advanced cases after the mean cell size became 400 μm² or greater. The corneal endothelial lesion thus became manifest, initially, by the appearance of just a few large cells. The latter became more numerous in number to appear in numerous clusters. In the later stage, these large cells accounted for a large proportion of endoth-elial cell population.
