We evaluated the corneal endothelium in 70 eyes of 35 cases who had worn contact lens for 2.5 to 25 years. We used specular microscope and mor-phometric analysis in the evaluation. We observed a decrease in endothelial cell density and an increase in the coefficient of variation (CV) of cell size along with the duration of contact lens wear. The increase in CV developed prior to the decrease in cell density. There was a positive correlation between the two, although the CV value never exceeded a value of 60.

The morphologic variables were independent of the state of fitting or the material of the contact lens. The decrease in endothelial cell population manifested two types: a cluster of large cells sur-rounded by groups of small cells and a moderate degree of pleomorphism with scattered large single cells.

The findings show that contact lens wearing may result in premature loss of corneal endothelial cells. CV changes are more pronounced than decrease in endothelial cell density.
