（B2-1-15）　浜松医科大学の関連病院14施設において，1993年3月から1998年2月までの5年間に経験した白内障術後眼内炎とその関連事項について調査した。約11,000件の白内障手術において5件（0.045％）の術後眼内炎が発生した。そのうち3件で原因菌が同定され，糖尿病患者が2件，鼻涙管閉塞の患者が2件あった。術前の抗菌薬点眼，涙嚢洗浄，睫毛切除，ポピドンヨードによる眼瞼消毒と結膜嚢洗浄の有無にかかわらず眼内炎の発症をみた。全例に硝子体切除術を行い，3例で眼内レンズの摘出を行った。術後視力は0.1から1.5であった。失明につながる白内障術後早期眼内炎に対処するためには，術後最低4日間は毎日の診察が望ましいと考えている。安易な抗菌薬の使用は慎むべきである。

We surveyed cases of endophthalmitis following cataract surgery in our institution and associated hospitals during the foregoing 5 years. Out of cataract surgery on consecutive 11,000 eyes, endophthalmitis developed in 5 eyes (0.045%). The latter included 2 diabetic patients and 2 cases of nasolacrimal duct stenosis. Causative organism could be identified in 3 cases. Incidence of endophthalmitis was dependent neither on instillation of antibiotics prior to surgery, irrigation of lacrimal sac, excision of cilia before surgery, asepsis of the eyelids, nor irrigation of the conjunctival sac. All the 5 eyes were eventually treated by vitrectomy. Intraocular lens was removed in 3 eyes. The final visual acuity ranged from 0.1 to 1.5. We advocate daily observation till the 4th postoperative day to detect initial signs of endophthalmitis. We also advocate against indiscriminate use of antibiotics in cataract surgery.
