真菌性眼内炎へのフルコナゾールの使用経験

Systemic fluconazol for endogenous fungal endophthalmitis

　真菌性眼内炎と考えられた3症例にフルコナゾールを使用した。症例1は75歳男性で直腸癌摘出後経中心静脈高カロリー輸液（IVH）が施行され，1か月後に飛蚊症を訴え両眼の赤道部に硝子体中に軽度に盛り上がった白色病巣を認めた。症例2は66歳男性。胆嚢摘出術3週間前からIVHが施行され，術後視力低下を訴え，両眼の後極部・周辺部網膜に黄白色の病巣を多数認めた。症例3は78歳男性，胃癌摘出後創部から真菌が検出されていた。3か月間特に訴えがなく初回眼底検査には異常なかったが3週間後に網膜内に限局した病巣を認めた。症例1は点滴，症例2,3では内服投与した。数週間で病巣は瘢痕化し再発は認めず，特別な副作用も認めなかった。

We treated 3 cases of endogenous fungal endo-phthalmitis with fluconazol. The patients were all males, aged 66, 75 and 78 years each. All had under-gone major abdominal surgery for cholecystitis, rectal cancer of gastric cancer. Two were placedunder intravenous hyperalimentation (IVH) during the postsurgical period. We treated one case with intravenous and 2 with peroral fluconazol. In all the cases, fundus lesions disappeared within 3 weeks after initiation of therapy without recurrences. Side effects of the therapy were absent throughout.
