高齢者の結膜嚢内常在菌と薬剤耐性

Bacterial floras and antibiotic susceptibility in the conjunctival sac in elderly persons

　眼感染症のない70歳以上の外来通院患者185例185眼を対象に，結膜嚢内常在菌とその薬剤耐性を検討した。101眼（54.6％）から細菌107株が検出され，93株（86.9％）はグラム陽性球菌であり，グラム陰性菌は7株（6.5％）と少なかった。また，半数以上の菌は薬剤耐性を有し，約20％の菌はキノロン系薬剤に耐性を示した。メチシリン耐性黄色ブドウ球菌（MRSA）は185眼中2眼（1.1％），多剤耐性コアグラーゼ陰性ブドウ球菌（MDRCNS）は3眼（1.6％）から検出された。結膜嚢内常在菌は眼感染症の起因菌となりうることから，抗菌剤の用い方には十分な注意が必要と考えられた。

We examined the bacterial organisms in the conjunctival sac in 185 eyes of 185 outpatients with no ocular infections. They were aged 70 years or over. A total of 107 bacterial strains were isolated from 101 eyes (55%) . They were Gram-positive cocci in 93 strains (87%) and Gram-negative in 7 (7%) . More than 50% of the strains were resistant to one or more antibiotics. About 20% of the strains were resistant to ofloxacin or norfloxacin. Methicillin-resistant Staphylococcus aureus (MRSA) was isolated in 2 out of 185 eyes (1.1%) and multiple drug-resistant coagulase-negative staphylococcus (MDRCNS) in 3 eyes (1.6%) . These features would merit due attention due to liability of bacterial floras in the conjunctival sac to develop ocular infections.
