Abstract Purpose:To compare the energy of Nd:YAG laser, the incidence of iris hemorrhage, the changes of intraocular pressure and corneal endothelial cell density between superior and inferior Nd:YAG laser iridotomy.

Methods:This study included 52 glaucoma patients who were scheduled to undergo Nd:YAG laser iridotomy. Mean age was 72.4±8.1 years and there were 36 females and16 males. Superior laser iridotomy was performed in 26 patients and inferior Nd:YAG laser iridotomy was carried out in 26 patients. Before June 2017, laser ridotomy was performed on the superior portion(11 to 1 o'clock)and after July 2017, on the inferior portion(5 to 7 o'clock).

Results:We used 5.3±2.2 Nd:YAG laser shots(34.5±18.2 mJ)in the superior group, and 4.0±1.6 laser shots(26.0±11.7 mJ)in the inferior group(p=0.0260). Iris hemorrhage was found in 6 patients(23%)in the superior group and 1 patient(4%)in the inferior group(p=0.0422). The one-year change of intraocular pressure and corneal endothelial cell density was−2.4±3.7 mmHg and−1.1±0.2% in the superior group and−1.6±4.0 mmHg and−1.2±0.3% in the inferior group, respectively;there was no difference between the two groups. No significant difference was noted for the incidence of laser iridotomy-specific linear dysphotopsia(p=0.2).

Conclusions:Inferior Nd:YAG laser iridotomy used significantly lesser energy and showed lower incidence of iris hemorrhage than superior iridotomy. There was no significant difference in the change in intraocular pressure and corneal endothelial cell density and the incidence of dysphotopsia.
