　ジヌソトミー（SIN）を併用したトラベクロトミー（LOT）の術後長期にわたる眼圧下降の機序を検討する目的で手術部位を超音波生体顕微鏡（UBM）を用いて観察した。対象は開放隅角緑内障44眼で，平均年齢54±19歳，術後平均観察期間27±12か月であった。LOT単独群を対照とし，細隙灯顕微鏡とUBMによる濾過胞の所見と眼圧との関係を検討した。その結果，今回の検索では，SINを併用したLOT （LOT＋SIN）においては細隙灯顕微鏡での濾過胞の大きさおよびUBMでの間隙と眼圧との相関性は明らかではなく，LOT＋SINの降圧効果は直接的な濾過作用のみに依存しているのではなく，これ以外の作用機序が関与している可能性が示唆された。

We evaluated the longterm course of filtering blebs in 44 eyes with open angle glaucoma following surgery by sinusotomy with trabeculotomy. The eyes were evaluated by slitlamp microscope and ultrasound biomicroscope (UBM) during the postoperative period of 27±12 12 months. The ages averaged 54±19 years. Another group of 13 eyes operated by trabeculotomy alone served as control. The postoperative intraocular pressure in the 44 eyes was correlated neither with the size of filtering blebs nor with the UBM findings of low-echo area. The findings show that the better effect of sinusotomy with trabeculotomy than trabeculotomy alone is not solely due to the direct filtering effect through the scleral flap but to other unknown mechanisms.
