超音波マイクロスコープによる小児ぶどう膜炎の経過観察

Follow-up of juvenile uveitis by ultrasound biomicroscopy

　超音波バイオマイクロスコープ（以下UBM）を用いて，炎症の程度を経時的に観察できた9歳女児の前部ぶどう膜炎を報告する。左眼のぶどう膜炎発症2週間後に当科を初診し，豚脂様角膜沈着物，高度な前房炎症，虹彩後癒着と視神経乳頭の充血を認めた。眼底周辺部は透見不能で，UBMを用いて毛様体の肥厚を認めた。ステロイド剤内服により前房の炎症は軽減した。半年後，左眼の併発白内障も進行し，前房炎症が再燃した。UBMで左眼の毛様体の肥厚に伴う水晶体と虹彩の前方移動が認められた。アトロピン点眼とベタメタゾンの結膜下注射により，前房炎症と毛様体肥厚が軽減した。UBMは前部および中間部のぶどう膜炎の経過観察に有用と思われた。

A 9-year-old girl presented with anterior uveitis in the left eye of 2 weeks' duration. The affected eye showed hyperemia, mutton-fat keratic precipitates, moderate flare and cells in the aqueous, and posterior synechia. The optic disc was hyperemic. No details of the retina could be seen. Ultrasound biomicroscopy, UBM, showed pronounced swelling of the ciliary body. Systemic corticosteroid led to recovery in 2 months. Six months later, anterior uveitis recurred with cataract. UMB showed swelling of the ciliary body in addition to anterior dislocation of the lens and the iris. Topical cycloplegics and corticoste-roid led to improvement in anterior uveitis and to resolution of the swelling of the ciliary body.
