Open sky vitrectomyを行ったMMC併用トラベクレクトミー後に発生した蜂巣炎を伴う眼内炎

Open-sky vitrectomy in an eye for endophthalmitis with orbital phlegmon following mitomycin C trabeculectomy

（V−33）　MMC併用トラベクレクトミー3年半後に，蜂巣炎を伴う重篤な眼内炎を発症した不幸な1例を経験した。抗生薬の全身および局所投与と硝子体手術を施行したが．光覚はもどらなかった。蜂巣炎を伴う重篤な眼内炎を発症した背景として，脆弱な濾過胞，糖尿病による易感染性，MMCによる強膜障害，強い毒性を有する病原菌の侵入などが考えられた。以上より，MMC併用トラベクレクトミー後の管理においては常に感染症を念頭におく必要があり，本症例のように易感染性疾患を有する場合には，特に注意が必要と思われた。

A 59-year-old diabetic female had received cataract extraction and trabeculectomy with mitomycin C in her left eye 41 months before. She developed ipsilateral endophthalmitis with orbital phlegmon since 2 days before. The affected eye resulted in blindness in spite of local and systemic antibiotics and vitreous surgery. This case illustrates that intraocular infection is a liability following mitomycin C trabeculectomy. It appeared that the endophthalmitis with poor final outcome was due to fragile bleb, presence of diabetes mellitus, impaired sclera by mitomycin C, and virulent microorgan-ism.
