We performed posterior chamber lens implanta-tion in 27 eyes (Group A) and additional trabeculectomy in 19 eyes (Group B). All the 46 eyes had cataract with glaucoma with intraocular pressure controlled below 21 mmHg with topical medication. The background factors were the same in both groups. Postoperative visual acuity of 0.5 or better was attained in 82% in Group A and in 53% in Group B. The difference was significant(p<0.05). Incidence of fibrin formation was less frequent (p < 0.05). Other postoperative complica-tion also tended to be less frequent. We calculated the possibility that the intraocular pressure control does not worsen using a life-table method of Ka-plan-Meier. It was 65±9% for Group A and 75± 11% for Group B. In Group B, significantly less medication was required postoperatively and signif-icantly lower intraocular pressure was attained at 1 -2 and 5-6 months. Posterior chamber lens im-plantation as a single procedure is thus indicated for cataract with mild glaucomatous damage.Additional trabeculectomy will benefit eyes with advanced glaucomatous damage.
