　70歳の男性に水晶体超音波乳化吸引術施行の際，後嚢破損のため眼内レンズ（IOL）を挿入せず，1年後に後房レンズ毛様溝縫着術を施行した。1年経過してIOLが脱落した。エンドスコープを用いIOL再縫着と眼内の観察を行った。IOLには混濁硝子体が絡んでおり，周辺部網膜は牽引性の剥離を起こしていた。充分な硝子体切除を行うことにより剥離網膜は復位し，経過良好である。後房レンズ毛様溝縫着術施行の際には，充分な硝子体切除，およびエンドスコープなどによる確実な毛様溝固定が必要であると思われた。

A 68-year-old male underwent phacoemulsifica-tion cataract surgery with unintentional rupture of posterior capsule. Secondary intraocular lens (IOL) implantation was performed 1 year later with the haptics sutured to the ciliary sulcus with 10-0 nylon thread. The IOL fell into the vitreous 1 year later.The dislocated IOL was removed and a new one was inserted with the aid of an endoscope. As vitreous opacity with tractional retinal detachment was detected during the procedure, vitrectomy was performed under endoscopic observation followed by resuture of the IOL haptics to the sulcus.
