　サルコイドーシスによる片眼性視神経乳頭肉芽腫の2例を経験した。症例1は17歳の女性で，左眼乳頭部に網膜浮腫を伴った肉芽腫様腫瘤がみられた。経気管支肺生検で非乾酪性肉芽腫があり，サルコイドーシスと診断された。症例2は65歳の男性で，右眼乳頭部に網膜出血を伴った肉芽腫様腫瘤がみられた。初期には全身的にサルコイドーシスの所見はなかったが，初診6か月後に血清アンジオテンシン変換酵素が異常高値を示し，経気管支肺胞洗浄でリンパ球増多がみられ，サルコイドーシスと診断された。2例とも副腎皮質ステロイド剤の全身投与が有効であった。乳頭部の原因不明の肉芽腫様病変ではサルコイドーシスを疑い，全身検索を繰り返す必要がある。

We observed unilateral granuloma of the optic nervehead due to sarcoidosis in 2 cases. The first case, a 17-year-old female, showed granulomatous mass on the disc with retinal edema in the left eye. Transbronchial lung biopsy showd noncaseous gra-nuloma and led to the diagnosis of sarcoidosis. The second case, a 65-year-old male, showed granulo-matous mass on the disc with retinal hemorrhage in his right eye. Systemic manifestations of sar-coidosis were initially absent. Sarcoidosis was diagnosed 6 months later by elevated serum level of angiotensin converting enzyme and presence of lymphocytosis by bronchoalveolar lavage. The findings suggest that repeated examinations for sarcoidosis are mandatory in patients with granu-lomatous lesions on the optic disc of unknown etiology.
