視神経乳頭小窩と神経線維層欠損の関連性

Relation between optic disc pit and retinal nerve fiber layer defect

（25C1-18）　過去15年間に視神経乳頭小窩と診断した自験例10症例を対象とし，視神経線維層欠損との関連性を検討した。男性5例，女性5例で，年齢は1〜64歳，平均31.7±13.5歳であった。神経線維層欠損は1歳の症例を除く9例で乳頭小窩の近傍に存在し，神経線維層欠損に一致した視野欠損が検出された。視神経乳頭小窩に神経線維層欠損が合併する原因として，乳頭小窩が生後の発育とともに次第に陥凹を増し．網膜が陥凹内へ牽引されることにより，視神経線維が伸展されて神経線維層欠損が生じると結論した。

We reviewed 10 cases of pit of the optic disc seen during the past 15 years. We paid particular attention to the association of retinal nerve fiber layer defect. The series comprised 5 males and 5 females. The age ranged from 1 to 64 years, average 31.7±13.5 years. Except for a one-year-old infant, 9 patients had nerve fiber layer defects near the disc and in the same sector of the optic disc pit. Corresponding visual field defect was also present. It is our interpretation that the retina is pulled and stretched towards the optic disc pit which excavates gradually with age, resulting in the nerve fiber layer defect.
