A 29-year-old woman presented with blurred vision in both eyes of 2 months' duration. Her corrected visual acuity was 1.2 in either eye. Both eyes showed cells in the aqueous,keratic precipitates,and iris nodules. Funduscopy showed slightly hyperemic optic disc. Tuberculin test was negative. Hypertrophy of hilar lymph nodes was bilaterally present,leading to the diagnosis of sarcoidosis. She discontinued treatment for 6 months. When she was seen by us 26 months after her initial visit,the corrected visual acuity was 1.2 right and 0.6 left. An optic disc granuloma was present in her left eye. It was 2 disc diameters across and was accompanied by serous retinal detachment. Two sessions of corticosteroid pulse therapy induced regression of granuloma. Uveitis has subsided for one year until present. This case illustrates that corticosteroid pulse therapy may be effective for optic disc granuloma in ocular sarcoidosis.
