サルコイドーシスによる視神経乳頭肉芽腫にステロイド剤パルス療法を行った1例

Pulse corticosteroid therapy in a case of sarcoidosis with optic disc granuloma

　サルコイドーシスによる視神経乳頭肉芽腫に対して副腎皮質ステロイド薬（ステロイド）のパルス療法により治療した。症例は22歳女性で，左眼の視朦感を主訴に当科を受診した。初診時の視力は両眼とも矯正1.0と良好。両眼に前部ぶどう膜炎と隅角結節，左眼には虹彩結節と視神経乳頭浮腫を認めた。眼所見よりサルコイドーシスを疑い，内科的検査を行ったところ，経気管支肺生検にてサルコイドーシスの確定診断が得られた。初診から2週間後に，左視神経乳頭上に肉芽腫様病変が出現し，徐々に増大，周囲に新生血管が発生した。ステロイドのパルス療法を行ったところ，乳頭肉芽腫は徐々に消退した。投薬は7か月で中止したが，肉芽腫の再燃はなく経過良好であった。

A 22-year-old female presented with blurred vision in the left eye. Visual acuity was intact when initially seen. Both eyes showed anterior uveitis and nodules in the chamber angle. The left eye showed iris nodules and optic disc edema. Lung biopsy led to the diagnosis of sarcoidosis. Granulomatous lesion with neovascularization started to grow from the disc in the left eye 2 weeks later. Systemic pulse therapy with methylprednisolone led to gradual disappearance of disc granuloma. No recurrence has been observed after discontinuation of 7 months of corticosteroid therapy.
