サルコイドーシスによる視神経乳頭肉芽腫

Granuloma in the optic nerve head due to sarcoidosis

　片眼の視神経乳頭に著しい乳頭腫脹を認め，乳頭の炎症性肉芽腫であった2症例を経験した。症例は12歳男性と52歳女性で，いずれも初診時には乳頭の腫瘍が疑われたが，臨床経過，とくにステロイド投与により治癒したので，乳頭の炎症性肉芽腫と診断した。原因疾患の確定はむつかしいが，臨床的に，サルコイドーシスと思われた。

　サルコイドーシスによる乳頭肉芽腫は最近報告が増えてきており，本邦における報告は現在までに6例を数える。視神経乳頭に腫瘤様病変を認めた時，肉芽腫を考え，サルコイドーシスを原因の一つとして考える必要があると思われた。

We observed inflammatory granuloma in the optic nerve head in a 12-year-old male and 52-year -old female. The condition was unilateral in both cases. While funduscopic findings suggested opticdisc tumor when first seen, both cases were diagnosed as granuloma of the optic disc. Fluores-cein angiography and prompt response to systemic corticosteroid led to the presumed diagnosis of sarcoidosis. The present cases are the 7th and 8th ones reported in Japan. Sarcoid granuloma is a possibility when a patient presents with mass lesion of the optic nerve head.
