Abstract 文献概要

　Trabeculotomy手術直後の併発症としての一過性眼圧上昇の予防策としてsinusotomyを併用する方法（LOT＋SIN）を用いてtrabecu-lotomy手術単独（LOT）の場合と比較検討したところ，以下の結果をみた。

　1）術後早期（24時間以内）に眼圧上昇をきたす症例が両群ともに認められたが，有意にLOT＋SIN群では少なく，30mmHg以上の眼圧上昇があった症例は，LOT＋SIN群で1眼（6％），LOT群で5眼（26％）であった。また術後6時間から24時間までの眼圧はLOT＋SIN群で有意に低く，LOT群では術後6時間に41mmHg，LOT＋SIN群では術後24時間に34mmHgが最高の眼圧であった。

　2）両群ともに，術後18時間に前房出血が多く残っている症例では，術後早期に眼圧が上昇していることが多い傾向がみられた。

　3） LOT＋SIN群では，濾過胞が嚢胞様の場合最終眼圧はlow teenthであった。

　以上の結果より，LOT＋SIN手術は術後早期の一過性眼圧上昇を抑え，視神経障害の進行した緑内障の症例に有川な手術であると考えられる。

We reviewed the effects of trabeculotomy and combined surgery with trabeculotomy and sinusotomy in open-angle glaucoma. Trabeculo-tomy was performed in 19 eyes and combined sur-gery in 18 eyes. The intraocular pressure (IOP) was measured 3, 6, 18 and 24 hours after surgery and later during the follow-up. There was no significant difference in IOP values at 3 hours and during thefollow-up. The IOP was significantly higher at 6,18 and 24 hours after trabeculotomy alone than the combined surgery. During the early postoperative period, trabeculotomy resulted in IOP of 30 mmHg or more in 5 eyes, 26%. The combined surgery resulted in elevated IOP in 1 eye, 6%. There were 2 eyes manifesting cystic filtering bleb after the combined surgery. The IOP during the later follow -up was within normal range.
