Pseudo-Exfoliation Syndrome, Effect of Trabeculotomy and the Scanning Eleciron Microscopic View of the Lens

緒言

　Pseudo-Exfoliation Syndrome （以下PE症候群と略記）は，水晶体表面の仮性落屑（第1図），虹彩縁への白色細片の付着，虹彩萎縮，隅角線維柱帯への色素沈着およびSampaolesi線等の所見を呈する一種の退行性変性疾患で，しばしば開放隅角緑内障を合併する。わが国においてもすでに多数の報告^1〜5）のある周知の疾患である。本症候群に伴う開放隅角緑内障は，単性緑内障にくらべ薬物治療に抵抗し，予後不良とされてきた^6〜9）。

　われわれは1972年初め頃より，手術治療を要する開放隅角緑内障に対してほぼ全例にTrabec—ulotomy ab externoを行なつてきたが，これらの症例の8例11眼が，PE症候群に合併したものであり，その眼圧調整成績はきわめて優秀であつた。

The purpose of this paper is to report the effect of trabeculotomy ab externo in the cases of open-angle glaucoma with pseudo-exfoliationsyndrome and to demonstrate the scanning electron micrograph of the surface of the lens in a case with this syndrome.

There were 17 cases (24 eyes) of open-angle glaucoma among 26 cases (40 eyes) of pseu-do-exfoliation syndrome during the period of recent 5 years.

Trabeculotomy was performed in 8 eyes and trabeculotomy was done simultaneously with cataract-extraction in 3 eyes. Trabeculotomy controlled the intraocular pressure without medication in 6 out of 8 eyes.
