原発開放隅角緑内障に対するサイヌソトミー併用トラベクロトミーの長期経過

Long-term outcome of trabeculotomy combined with sinusotomy for primary open-angle glaucoma

要約　1996年までの6年間に，サイヌソトミーを併用するトラベクトミーを原発開放隅角緑内障に対する初回手術として行い，5年以上経過を追うことのできた64例83眼を検討した。手術時の平均年齢は57.5±12.3歳，術後の観察期間は7.2±1.5年であった。術前の平均眼圧は23.6±4.6mmHg，術後5年目のそれは15.4±2.4mmHgであり，有意に下降していた（p＜0.001）。生命数理法による20mmHg以下への生存率は0.68±0.05（累積生存率±標準誤差）であった。以上より，本術式では術後長期間にわたって有意な眼圧下降が維持され，有用な治療手技であると結論される。

Abstract.　We reviewed 83 eyes of 64 patients who received trabeculotomy combined with sinusotomy for primary open-angle glaucoma during 6 years through 1996 and who could be followed up for 5 years or longer. The age at surgery averaged 57±12.3 years. The follow-up period averaged 7.2±1.5 years. The intraocular pressure averaged 23.6±4.6mmHg before surgery and 15.4±2.4mmHg 5 years after surgery. The difference was significant（p＜0.001）. By acturial life table method,the success rate below 20 mmHg was 0.68±0.05. The findings show that the current surgical modality is clinically useful in inducing prolonged control of intraocular pressure.
