先天性乳頭上硝子体嚢腫の1例

A case of congenital prepapillary vitreous cyst

　71歳，女性の右限視神経乳頭上に認められた先天性乳頭上硝子体嚢腫について報告した。嚢腫は直径1乳頭径大で視神経乳頭に付着して存在し，硝子体ゲルとは後硝子体膜の中央部で連がっており眼球運動によるゲルの動きとともにわずかに振動した。嚢腫の存在位置と硝予体ゲルとの関係から硝子体管の遺残物として形成された先天性乳頭上硝子体嚢腫と診断した。先天性乳頭上硝子体嚢腫は極めて稀であり本邦では最初の報告と考えられた。

A 71-year-old female had noted myodesopsia inher right eye for the past 10 years. A globular cyst, about 1 disc diameter in size, was found in front of but a little lower nasal to the optic disc in the right eye. The cyst was greyish and semitransparent. It was apparently avascular and was not associated with inflammatory sings in or around. The vitreous had underwent posterior vitreous detachment with collapse. The cyst was attached to the detached posterior vitreous membrane. During saccadic eye movement, the cyst was almost fixed to the disc and trembled through the pull by the vitreous.
