　鈍的外傷後眼内レンズ脱出および裂孔性網膜剥離を生じた1例を経験した。

　症例は82歳女性で，右眼のECCEおよび眼内レンズ挿入術施行後8か月目に転倒して，机に右眼を打撲。翌日，脱出した眼内レンズを持参して再受診した。右眼視力は光覚弁で虹彩脱出・前房出血があり眼底は透見不能であった。同日，創口閉鎖術施行。その後網膜剥離が認められたため硝子体切除術を施行した。この術中，裂孔原性網膜剥離が明らかとなり裂孔閉鎖術を行った。術後4か月の現在，視力は（0.03）で網膜は復位しており経過観察中である。術前および術中所見・受傷機転から裂孔の発生機序として脱出時のレンズのループの網膜への直接の損傷が考えられた。

An 82-year-old female underwent uneventful extracapsular cataract extraction with posterior chamber lens implantation in her right eye. Eight months later, she slipped and was hit in the right forehead. The intraocular lens was extruded. When examined the following day, the corneoscleral wound was ruptured through its entire length with incarceration of the iris. The anterior chamber was filled with blood and fibrinous exudate. We irrigat-ed the anterior chamber and resutured the wound. Signs of retinal detachment became manifest 2 weeks after the injury. We performed simple vitrectomy 2 days later. The retinal detachment was apparently due to retinal tear formation with the inferior loop of the lens was extruded during the injury.
