糖尿病性牽引性網膜剥離眼に対する硝子体手術成績

Results of vitrectomy for traction retinal detachment in diabetic retinopathy

　1981年3月より1983年8月までの問に硝子体切除術を行った糖尿病性牽引性網膜剥離眼69例86眼の手術成績について報告する。

　硝子体に対する手術操作に加えて60眼（69.8％）に輪状締結術を行い，26眼（30.2％）に強膜切除術を行った。更に術前の光凝固治療が不充分であった29眼（33.7％）では経強膜網膜冷凍凝固を同時に行った。

　術後，52眼（60.5％）で視力が2段以上改善，19眼（22.1％）で視力が2段以下に低下，失明した症例は12眼（14.0％）であった。

　視力不変群および視力低下群の中で視野の改善が認められた症例が14眼あり視力改善群と合せ考えると，計66眼（76.7％）に視機能の改善が認められた。

　We performed pars plana vitrectomy on 86 eyes with diabetic traction retinal detachment during the 30-month period ending August 1983. The macula was also detached preoperatively in 80 eyes.

　In addition to simple vitrectomy and membranectomy with Ocutome, membrane peeling, segmentation and delamination was done in 52 eyes (61%). Scleral surgery was also performed in all cases, either by encircling buckling procedure(60 eyes) or scleral resection (26 eyes). Transscicral retinal cryopexy was performed simultaneously in cases with insufficient presurgical photocoagulation (26 eyes).
