　過去7年間に初回に硝子体手術を行った眼内レンズ挿入眼の網膜剥離の成績を評価した。硝子体切除術単独が7眼，周辺硝子体牽引除去を目的に強膜内陥術を併用したのが15眼であった。20眼（91％）で初回に復位が得られた。再手術を必要とした2眼は，いずれも硝子体切除単独例であった。最終的に21眼（95％）で復位が得られた。術後視力は，2段階以上の改善が18眼（82％），悪化が2眼（9％），不変が2眼（9％）であった。術後合併症として，瞳孔捕獲と黄斑パッカーが各1眼，増殖硝子体網膜症が2眼に生じた。眼内レンズ挿入眼の網膜剥離に対して初回手術としての硝子体手術が有効であることを示す症例群である。

We reviewed 22 eyes of 21 cases of pseudophakic retinal detachment treated by pars plana vitrectomy as the initial procedure during the past 7 years. Out of the 22 eyes, 15 eyes received additional scleral buckling to reduce peripheral vitreal traction. Retinal reattachment was obtained in 20 eyes (91%) after the first procedure. Multiple surgeries had to be performed in 2 eyes after simple vitrectomy. Final reattachment was obtained in 21 eyes (95%). Visual acuity improved by 2 lines or more in 18 eyes, decreased by 2 lines or more in 2 eyes and remained unchanged in 2 eyes. Complications included pupillary capture 1 eye, macular pucker 1 eye and proliferative vitreoretinopathy 2 eyes. The findings show the efficacy of vitrectomy as the primary procedure for pseudophakic retinal detachment.
