一過性黒内障を再発し網膜中心動脈閉塞症を発症した内頸動脈閉塞症

Recurrent amaurosis fugax and eventual central retinal artery occlusion in internal carotid obstruction

　網膜中心動脈閉塞症発症後，内頸動脈閉塞症が発見され，視力改善を見た58歳の男性症例を報告した。症例は初診までに7回の黒内障発作を経験していた。初診時右眼の軽度の視力低下と眼底の高血圧性および動脈硬化性変化を示していたが，その5日後，右眼視力は0.03に低下し，眼底は浮腫状になった。螢光眼底撮影にて著しい循環時間遅延が見られ，乳頭黄斑線維束は一部毛様網膜動脈にて灌流されていた。このことから中心動脈閉塞症と診断した。頸動脈造影で同側の内頸動脈閉塞症が発見された。入院後の保存的療法により視力は最終的に1.0に改善した。再発する黒内障発作および網膜中心動脈閉塞症の基礎疾患として，内頸動脈閉塞症を考慮すべきと考えた。

A 58-year-old male presented with blurring of vision in his right eye. There had been 7 recurrent episodes of amaurosis fugax during the foregoing 4-year period. He developed, 5 days later, acute central retinal artery occlusion in the right eye withcompatible funduscopic and fluorescein angiogra-phic features. A cilioretinal artery irrigating the macula was spared from the occlusion. The visual acuity was 0.03 on day 1 of the disease and recover-ed to 1.0 eventually. We detected obstruction of right internal carotid artery at the site of bifurca-tion as the underlying cause of amaurosis fugax and central retinal artery occlusion.
