内頸動脈閉塞に続発する緑内障

Secondary Glaucoma Following Carotid Occlusion

I．緒言

　内頸動脈閉塞は近年，眼科医にとつても注目の疾患となつてきている。その眼科的症状の故のみならず，眼底血圧，Ophthalmodynamographie（Hager），頸動脈圧迫トノグラフィー試験（Rod—riguez-Barrios），螢光眼底検査など，診断上にはたす眼科の役割からである。

　内頸動脈閉塞においては眼動脈圧の低下とともに，同側の眼圧は，特に初期には一般に低下している。そこに片側性の緑内障が起こるとは一見奇異に感じられるが，この型の緑内障は従来あまり知られていない。最近この種の患者2例を経験したが，内頸動脈閉塞に遭遇する機会が増大する傾向に鑑み，ここに簡単に報告して，この特殊の緑内障に注目を促したい。

A description is presented on two cases (55-year-old male and 60-year-old male) of second-ary glaucoma which developed after an occlu-sion and a stenosis of the ipsilateral internal carotid artery. It was a kind of hemorrhagic glaucoma with new vessels growing over the anterior surface of the iris and across the angle of the anterior chamber. Rubeosis of the iris is a rare occurrence after obstruction of central retinal artery. A reduction in facility of aqueous outflow following carotid occlusion seems to have played a role in the development of glaucoma in the present cases.
