一過性黒内障で発症した内頚動脈閉塞症の1例

Internal carotid artery stenosis developed in a case of amaurosis fugax

　63歳の男性で左眼の一過性黒内障を初発として眼科を受診し，その後急速に脳梗塞に至った内頚動脈狭窄症の1例を経験した。左眼に低眼圧と眼底後極部の浮腫，多数の動脈分岐部の栓子，および蛍光造影で脈絡膜の充盈遅延と動静脈のsludgingが観察された。神経学的異常や頚部血管雑音は聴取されなかったが左内頚動脈狭窄を疑い，ウロキナーゼの点滴等の治療及び精査を開始した。しかし点滴開始後6日目に左脳梗塞を発症した。半年後の頚動脈造影では左内頚動脈起始部の狭窄と左眼動脈の狭窄を認め，内頚動脈狭窄部からの塞栓により眼動脈，ついでM₁M₂部の閉塞を来したと考えられた。本例より眼症状だけでも内頚動脈狭窄を疑うときには，頚動脈造影による確定診断と脳外科的治療の適応の検討を速やかに行うべきと反省させられた。

A 63-year-old male presented with clinical signs of left amaurosis fugax. We observed low intraocular pressure, edema in the posterior retina and numer-ous cholesterol plaques at the branchings of retinal arteries in the affected eye. Fluorescein angiogra-phy showed delayed choroidal circulation and slud-ging in retinal arteries and veins. While the patient was free of systemic signs, we suspected stenosis of left internal carotid artery.

On day 6 of treatment with systemic urokinase, he developed left cerebral infarction. Carotid angio-graphy 6 months later showed stenosis of left inter-nal carotid and ophthalmic arteries. It appeared that the embolus from the stenosed internal carotid initially occluded the left ophthalmic artery and later the middle cerebral artery.
