要約　目的：大腸癌術後患者において発症した明らかな高次脳機能障害を有さない皮質盲の1例を経験したので報告する。

症例：67歳，男性。横行結腸癌に対して腹腔鏡下結腸切除術を施行された。1か月後に視力障害を家族に指摘され，福岡大学病院眼科を紹介され受診となった。初診時視力は右（0.08），左（0.1），眼圧は右19mmHg，左16mmHgであった。前眼部および眼底には明らかな異常はなく，両眼ともに下方の非特異的な視野欠損を認めた。失語や失認，失行などの高次脳機能障害は認められなかった。頭部単純CT検査で後頭葉に低吸収域を認めたために頭部MRI検査（拡散強調画像）を施行したところ，両側後頭葉に急性期脳梗塞を認めて皮質盲と診断された。視力障害の自覚に乏しいことからアントン徴候は陽性であった。原因としてアテローム血栓性脳梗塞を認め，アスピリン内服とヘパリン持続点滴で加療された。治療後に右眼の視野欠損は軽度改善したが，視力改善はみられなかった。

結論：失語や失認などの高次機能障害を認めない稀な皮質盲を経験した。非特異的な視野異常や視力障害がみられる場合には，積極的に頭部MRI検査（拡散強調画像）を行うべきである。

Abstract　Purpose：To report a case of cortical blindness in a patient with cancer without severe higher cerebral dysfunction.

Case：A 67-year-old male underwent laparoscopic colon resection for transverse colon cancer. One month post-surgery, the patient's family noted the onset of visual impairment. On examination, the patient's visual acuity was 0.08 and 0.1 on the right and left respectively. Intraocular pressure was 19 and 16 mmHg on the right and left respectively. No abnormalities were identified in the anterior segment or fundus of the eyes. The visual field showed nonspecific defects in the inferior regions of both eyes. No higher functional impairment such as aphasia, apraxia, or speech apraxia. Computed tomography（CT）of the head showed hypo-absorption areas in the both occipital lobes. Magnetic resonance imaging（MRI）of the head（diffusion-weighted imaging）demonstrated acute cerebral infarction in the bilateral occipital lobes, leading to a diagnosis of cortical blindness. Anton's sign was positive due to the patient's ignorance of blindness by their patient. The cerebral infarctions were a cause of cortical blindness. After medical treatment, his visual field defects improved slightly, but his visual disturbances have not improved since.

Conclusion：Cortical blindness rarely occurs without an apparent higher cerebral dysfunction. We recommended performing a diffusion-weighted MRI of the head in patients with nonspecific visual field defects or visual impairment.
